After reviewing the above guidelines, please download this form, sign it, and email it back to me (sohayla@anastaziaskincare.com). This must be done at least 72 hours prior to your peel appointment.
COMPLETION OF THIS FORM IS REQUIRED FOR YOUR PEEL TREATMENT
